Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Manitowoc, WI. The PRN role supports the Physical Therapist in delivering resident-centered care.

Responsibilities include screenings, evaluations, and both individual and group therapy focusing on mobility, gait, and strength, with CMS documentation in NetHealth EMR. You will educate residents and families while collaborating with the care team to ensure quality outcomes.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations and in developing target goals and treatment plans with the Physical Therapist.
• Conduct both individual and group treatments as outlined in the treatment plan focusing on strength, mobility, gait, ROM and balance.
• Monitor residents' response to intervention and consult with the Physical Therapist for modifications.
• Assist in establishing resident schedules with the Physical Therapist and team members.
• Educate residents, families, and appropriate staff in equipment use and safe practices.
• Document CMS-compliant, medically necessary care in NetHealth EMR.
• Participate in Quality Assurance activities as required.
