Job summary

Evolve Therapy Services, LLC in Florida is seeking a licensed Physical Therapy Assistant for an in-house Skilled Nursing Facility. You will deliver PT services under the supervision of a Physical Therapist and participate in resident screenings, treatment planning, and education of residents, families, and staff.

The role emphasizes adherence to CMS guidelines, documentation in NetHealth EMR, and collaboration with the care team to maintain high-quality resident care. APTA guidelines encouraged.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.
• CMS/Regulatory guidelines followed in daily practice.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the Physical Therapist.
• Assist in developing target goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatment focusing on strength, mobility, balance, gait, ROM and modalities.
• Monitor residents’ response to treatment and adjust plans with the Physical Therapist.
• Maintain resident schedules and coordinate with the care team.
• Educate residents, families and staff on therapies and equipment usage.
• Document compliant and medically necessary notes in NetHealth EMR as CMS requires.
