Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility located in Hopkins, MN. This full-time or PRN position requires licensure in the state and a BS in Physical Therapy, with emphasis on planning, evaluating, and directing therapy services in a CMS-regulated setting.

Responsibilities include evaluation, treatment planning, documentation, and collaboration with the interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure/certification.
• Commitment to ongoing professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents per standards and facility procedures.
• Complete documentation and billing for physical therapy services per CMS and NetHealth EMR requirements.
• Evaluate and plan resident therapy programs with short- and long-term goals; communicate results to families and team.
• Conduct regular resident screenings to determine intervention needs and track progress.
• Develop treatment plans and adjust as needed to achieve identified goals.
• Interpret evaluation findings and treatment plans to residents, families, and interdisciplinary team.
