Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages. The role includes designing patient-centered plans of care, educating patients and families, and monitoring outcomes. A sign-on bonus is offered and relocation is available for eligible candidates.

Requirements include a Bachelor's Degree in Physical Therapy and licensure as a Physical Therapist. BLS must be obtained within 30 days.

Qualifications

• Bachelor's Degree in Physical Therapy
• Licensed Physical Therapist (PT)
• Basic Life Support (BLS) within 30 days

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor; uses safe body mechanics.
• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan; adjusts based on patient response.
• Educates patients, families, caregivers, students, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Completes examination selecting reliable methods relevant to chief complaint and history.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine need for further therapy services; uses therapy EMR for daily documentation and charging.
• Completes annual education to comply with DNV Hip and Knee Replacement Program requirements.
• Evaluates impairments related to orthopedic and neurologic conditions; establishes plans of care; completes ROM/strength tests; administers modalities.
• Communicates progress and recommendations to referring providers.
