Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, adjusting care plans based on response and evidence. This role includes a sign-on bonus and relocation assistance for eligible candidates.

The successful candidate will provide patient-centered therapy, educate families and staff, and document outcomes in the EMR. A BLS certification and current license are required, with ongoing CEUs to meet program standards.

Qualifications

• Licensed physical therapist in good standing
• BLS certification within 30 days of hire
• Experience or training in acute stroke care and rehab

Responsibilities

• Evaluates patients and develops an integrated plan of care including goals, treatment, outcomes, and discharge planning.
• Educates patients, families and staff using effective teaching methods.
• Performs examinations and selects appropriate tests to determine diagnosis and progress.
• Documents care in the EMR and maintains accurate daily notes and billing.
• Communicates progress and recommendations with referring providers and care team.
