Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, developing and modifying plans of care with patient-specific goals. This role serves as a clinical resource and model for staff and students, with a sign-on bonus and potential relocation support.

The ideal candidate holds a valid PT license (IDFPR), BLS certification, and is committed to ongoing education, including acute stroke care.

Qualifications

Licensed PT (IDFPR) in good standing.
BLS within 30 days - AHA.
Continuing education in acute stroke care.

Responsibilities

Performs patient transfers with safe body mechanics to minimize injury.
Treats patients with empathy and maintains engagement with those needing services.
Designs and adjusts plan of care based on goals, treatment, outcomes and discharge plan.
Educates patients, family, caregivers, students and other health care providers.
Evaluates clinical findings to determine PT diagnoses and outcomes.
Performs examination using valid methods relevant to chief complaint and history.
Documents in the EMR per policy CTSCP67.
Screens patients to determine need for further therapy services.
Supervises support personnel and provides feedback as needed.
Completes annual education to comply with HIP/Joint replacement program.
