Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care as needed. The role offers a sign-on bonus and relocation assistance for eligible candidates.

You will design care plans, educate patients and families, perform examinations, and document in the EMR. Active PT licensure and BLS within 30 days are required, with ongoing CEU participation.

Qualifications

• Licensed Physical Therapist (PT) in good standing
• BLS within 30 days - AHA
• Attend and satisfactorily complete continuing education on acute stroke care

Responsibilities

• Evaluates and treats patients of all ages, modifying plan of care as appropriate.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes and discharge plan.
• Educates patients, family, caregivers, students and other health care providers.
• Evaluates clinical findings to determine diagnoses and outcomes of care.
• Performs examinations using appropriate methods relevant to chief complaint and history.
• Documents in EMR per policy.
• Screens patients to determine therapy needs and uses support personnel per standards.
