Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages in our facility. You will modify plans of care, educate patients and families, and document outcomes in the EMR. The role offers a sign-on bonus and relocation assistance for qualified candidates.

Qualifications

• Licensed Physical Therapist (PT) eligible for licensure in the applicable state.
• Basic Life Support (BLS) certification within 30 days.
• Complete required continuing education related to acute stroke care.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed and maintains safe body mechanics.
• Designs and adjusts an individualized plan of care with goals, treatment, outcomes, and discharge plan.
• Educates patients, families, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes, and documents in the EMR.
