Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, developing and modifying plans of care. You will serve as a clinical resource for staff and students and participate in a multidisciplinary team to achieve optimal outcomes.

Responsibilities include performing examinations, establishing care plans with measurable goals, educating patients and families, documenting progress in the EMR, and ensuring safety with proper body mechanics.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) per IDFPR.
• BLS certification within 30 days (AHA).
• Complete required continuing education for acute stroke care.

Responsibilities

• Performs patient transfers with safe body mechanics.
• Develops and adjusts plan of care with goals and outcomes.
• Educates patients and families using effective teaching methods.
• Evaluates findings to determine PT diagnoses and outcomes.
• Documents in the therapy EMR and conducts objective testing.
