Job summary

Carle Health is seeking a Licensed Physical Therapist (PT) to evaluate and treat patients of all ages, developing and adjusting plans of care based on patient response and evidence. The role involves education for patients and caregivers, thorough documentation, and collaboration with a multidisciplinary team.

New grads may qualify for a sign-on bonus, with relocation assistance available for those moving 50+ miles.

Qualifications

Bachelor’s Degree in Physical Therapy required.
Licensed Physical Therapist (PT) in good standing.
Basic Life Support (BLS) certification within 30 days.

Responsibilities

Demonstrates ability to perform patient transfers with assistance as needed, using safe body mechanics.
Designs an appropriate plan of care that integrates goals, treatment, outcomes and discharge plan.
Educates patients, family, caregivers and other health care providers.
Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
Performs examinations with reliable methods relevant to chief complaint and history.
Produces documentation in accordance with CTSCP67 and related policies.
Screens patients to determine need for further therapy services and monitors care by staff.
Applies therapeutic modalities and uses support personnel per guidelines.
Communicates progress and treatment recommendations to referring providers.
Evaluates impairments related to orthopedic and neurologic conditions to establish plans of care.
