Job summary

Carle Health is seeking a qualified Physical Therapist to evaluate and treat patients of all ages in our rehabilitation services. The role emphasizes safe transfers, outcome-driven plans of care, and patient education.

You will perform examinations, document in the EMR, and coordinate with providers. A BLS certification and state license are required, with ongoing education for acute stroke care as applicable.

Qualifications

• Bachelor’s degree in Physical Therapy or equivalent.
• Licensed Physical Therapist (PT) in good standing (IDFPR)
• BLS certification within 30 days of start
• Continuing education for acute stroke care as required

Responsibilities

• Performs patient transfers with safe body mechanics.
• Delivers patient-centered care with dignity and empathy.
• Develops and adjusts a plan of care with goals and discharge plan.
• Educates patients, families, and staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Conducts examinations with reliable methods based on history and screening.
• Documents care per Clinical Process Policy (CTSCP67).
• Screens patients for therapy needs and responsiveness.
• Directs support personnel per legal/ethical guidelines and provides feedback.
• Completes annual education for DNV Hip and Knee Programs when applicable
• Evaluates impairments related to orthopedic and neurologic conditions
• Establishes plan of care based on deficits and goals
• Performs ROM, strength, and functional testing
• Documents daily in therapy EMR
• Applies tapes for stabilization or facilitation
• Administers modalities (ultrasound, interferential, iontophoresis)
• Communicates progress with referring providers
