Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a Part Time Physical Therapist (PT) opening at River Oaks Healthcare Center. This role keeps you as an in-house team member and offers a chance to impact residents through therapy while serving at multiple centers across WV.

A PT credential and license are required or pending. You’ll collaborate with the rehab team, lead treatment plans, and participate in interdisciplinary meetings.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
