Job summary

CommuniCare Rehab is seeking a PRN Physical Therapist (PT) at Webster Healthcare Center in Cowen, WV. You will evaluate patients, develop and implement treatment plans, and supervise PTAs while collaborating with an interdisciplinary rehab team.

With a long-standing heritage since 1984, CommuniCare emphasizes person-centered care across multiple facilities, offering ongoing CEU resources and stable, in-house roles for qualified therapists.

Qualifications

• Bachelor's/Master's/Doctorate in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patients' medical charts and develop a treatment plan.
• Implement treatment plan directly and educate the plan of care to the rehab team.
• Attend and lead interdisciplinary meetings to relay findings and recommend actions.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
