Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, is hiring a Physical Therapist (PT) for a full-time role at Berkeley Springs Healthcare Center. A $15,000 sign-on bonus accompanies competitive wages and a supportive in-house team environment.

The PT will evaluate patient charts, develop and implement treatment plans, supervise PTAs, and participate in interdisciplinary meetings to enhance resident care. Opportunities exist to serve at multiple centers.

Qualifications

• A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
