Job summary

CommuniCare Rehab is seeking a Physical Therapist for a full-time split role serving Seneca Trail Healthcare Center and Lewisburg Healthcare Center. You will evaluate patient charts, develop and implement treatment plans, supervise PTAs, and lead interdisciplinary meetings.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an Accredited Program.
• State Physical Therapy License must be valid or pending.

Responsibilities

• Evaluate, review, and screen patient medical charts and develop a treatment plan.
• Implement plan through direct treatment and supervise PTAs as needed.
• Lead interdisciplinary meetings and educate rehab team and families when appropriate.
• Work across multiple centers to serve diverse patient needs.

Qualifications

• A Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an Accredited Program
• Hold a valid or pending State Physical Therapy License

Responsibilities

• Evaluate, Review, and Screen patient's/resident's medical charts, evaluations under physician order and develop a treatment plan in accordance with patient's needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
