Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a full-time opening for a Physical Therapist (PT) at Wellsburg Healthcare Center, where our therapists are in-house team members.

As a PT, you will evaluate patients, develop treatment plans, and collaborate with the rehab team to deliver high-quality care across our centers, improving resident outcomes and maintaining continuity of care.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an Accredited Program.
• Valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patient charts and develop a treatment plan.
• Implement treatment plan through direct care and supervise PTAs.
• Lead interdisciplinary meetings and relay clinical findings to rehab team; provide staff/family education.
• Opportunity to work at multiple centers to serve diverse patient needs.

Qualifications

A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program. Hold a valid or pending State Physical Therapy License

Job Responsibilities

Job Responsibilities include, but are not limited to: Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs. Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs. Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
