Job summary

CommuniCare Rehab is hiring a Physical Therapist (PT) for Clinton Healthcare Center in Clinton, MD. This full-time role features an in-house team environment and opportunities to work at multiple centers across the network.

You will evaluate, screen and treat patients, develop plans of care, supervise PTAs, and participate in interdisciplinary meetings to share findings with the rehab team. Commitment to high-quality, person-centered care is essential.

Qualifications

• Licensed or eligible for licensure in Maryland
• PT degree from an accredited program

Responsibilities

Qualifications:

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs. Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs. Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
