Job summary

CommuniCare Rehab in Berkeley Springs, WV is seeking a Physical Therapist (PT) to evaluate, plan, and implement patient care across our rehab teams. This part-time in-house role offers opportunities to work with residents and contribute to individualized treatment plans.

You will collaborate with multidisciplinary staff, review charts, provide supervision to PTAs, and participate in meetings. Various centers allow serving a diverse range of patient needs while maintaining work-life balance.

Qualifications

• PT degree from an accredited program.
• Holds a valid or pending state PT license.

Responsibilities

• Evaluate, review, and screen patient charts and evaluations under physician order and develop a treatment plan.
• Implement treatment plan through direct treatment and educate the plan of care to all team members, including supervising PTAs.
• Lead interdisciplinary meetings to relay clinical findings and make rehab recommendations; provide staff/family training when appropriate.
• Opportunity to work at multiple centers to serve a wide variety of patient needs.

Education

Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
