Job summary

Ascension's Alexian Brothers Rehabilitation Hospital in Elk Grove Village, IL, seeks a full-time Physical Therapist to deliver outpatient rehab across neuro, ortho, and specialty areas. You’ll develop and reassess patient-centered plans with families, collaborate with an interdisciplinary team, and support safe discharge planning.

Schedule includes two evenings weekly until ~7:15 p.m. and daytime hours otherwise.

Qualifications

• BLS certification within 30 days of hire.
• Illinois PT license required/credentialed prior to hire (license-pending accepted).
• Bachelor's degree required; Master's/Doctorate preferred.

Responsibilities

• Provide comprehensive rehabilitative care for a clinically diverse patient population, including neuro, ortho, and specialty areas.
• Develop and implement individualized, patient-centered plans of care in collaboration with patients and families.
• Deliver evidence-based interventions across complex diagnoses and coordinate with the interdisciplinary team.
• Collaborate with the team to optimize outcomes and support safe, effective discharge planning.
• Schedule includes two evenings per week until ~7:15 p.m.; other days 7:00 a.m.–3:30 p.m.

• Schedule: Rotating shift | Full-time

How you’ll make an impact in this role

• Provide comprehensive rehabilitative care for a clinically diverse patient population, specializing in a wide cross section of neuro, ortho, and specialty areas like lymphedema and pelvic floor.
• Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families.
• Deliver evidence-based interventions across complex diagnoses, including neurological, oncological, orthopedic, cardiopulmonary, and medically complex conditions.
• Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.
• Schedule will have 2 evenings a week until approximately 7:15 p.m. The other days are 7 a.m. to 3:30 p.m.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Physical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.

Education

• Bachelor's degree required. Master's OR Doctorate degree preferred.
