WelbeHealth is seeking a Physical Therapist to join the interdisciplinary team in New Jersey. You will assess participants, participate in IDT planning, and deliver and document restorative interventions to maximize independence.

Qualifications

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred.
• Valid New Jersey Physical Therapy License, or proof of license eligibility.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Education

Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
