Job summary

WelbeHealth is seeking a Physical Therapist to join our interdisciplinary team, focusing on restorative and rehabilitative therapy to maximize independence and safety for participants. You will work within the IDT, deliver and document interventions, and support care plans aligned with patient goals.

Qualifications

• Bachelor’s degree in physical therapy, master’s or doctoral degree preferred.
• Valid California Physical Therapy License or license eligibility.
• CPR and First Aid certification within 180 days of hire preferred.

Responsibilities

• Conduct Physical Therapy assessments in the facility and home as needed.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per participant care plans.
• Provide active and passive exercises, gait and ADL training, and therapies.
• Supervise Therapy Assistants and guide Functional Maintenance Programs.
• Instruct staff, participants, and caregivers on therapeutic equipment use.
