Job summary

WelbeHealth is seeking a Physical Therapist to join our interdisciplinary team (IDT) and deliver restorative and rehabilitative therapy to participants, coordinating care planning and ensuring independence and safety. You will perform assessments, participate in IDT meetings, document interventions, supervise therapy assistants, and train staff on therapeutic equipment.

CA PT license and degree required; outpatient/acute/inpatient experience preferred.

Qualifications

• Bachelor’s degree in physical therapy; Master’s or Doctoral degree preferred.
• Valid California Physical Therapy License or license eligibility.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct PT assessments in the facility and occasional home visits.
• Actively participate in IDT meetings to develop care plans.
• Deliver and document interventions as per participant care plans.
• Provide active and passive exercises, gait training, and ADL training.
• Supervise Therapy Assistants and train staff on equipment use.
