WelbeHealth, LLC. seeks a Physical Therapist to join our interdisciplinary team delivering patient-centered care across clinic and home settings. You will perform assessments, develop care plans, and help enable independence for participants through restorative therapies.

The role emphasizes long-term relationships, collaborative care, and a balanced workload with comprehensive benefits. Requires CA PT license and CPR/First Aid credentials.

Qualifications

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred.
• Valid California Physical Therapy License, or proof of license eligibility.
• CPR and First Aid certification within 180 days of hire (preferred on hire).

Responsibilities

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment.
