Job summary

WelbeHealth seeks a Physical Therapist to join our interdisciplinary team focused on patient-centered care for participants. You will plan and coordinate restorative therapies to maximize independence and safety, while actively contributing to IDT care plans.

Qualifications

• Bachelor’s degree in physical therapy; master’s/doctoral preferred.
• Valid California Physical Therapy License or proof of eligibility.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct Physical Therapy assessments in our facility and occasionally in the home.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, gait, ADL training, and modalities.
• Supervise Therapy Assistants, training staff to implement maintenance programs.
• Provide training to staff and caregivers on therapeutic equipment use.
