Job summary

WelbeHealth, LLC. is seeking a Physical Therapist to join our interdisciplinary team and deliver restorative and rehabilitative PT services to participants, focusing on independence and safety.

The role emphasizes long-term relationships with participants and collaboration with therapists, providers, and disciplines. Compensation includes base pay plus bonus, comprehensive benefits, and a supportive work-life balance with extensive PTO and holidays.

Qualifications

• Bachelor's degree in physical therapy; master's or doctoral degree preferred.
• Valid California Physical Therapy License, or proof of license eligibility.
• CPR and First Aid certification within 180 days of hire and maintained thereafter.

Responsibilities

• Conduct PT assessments in facility and occasionally in participant homes to define care needs and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document interventions per care plans.
• Provide active/passive exercise, gait, ADL training, and modalities (massage, heat, sound, water, light, or electricity).
• Supervise Therapy Assistants and train staff on Functional Maintenance Programs.
• Educate participants, staff, and families on therapeutic equipment care.
