WelbeHealth LLC is seeking a Physical Therapist to join our interdisciplinary team and deliver restorative and rehabilitative physical therapy services to participants. You’ll focus on care planning, maximize independence, and collaborate with therapists and providers to ensure comprehensive care.

The role emphasizes long-term relationships with participants, balanced caseloads, and opportunities to work in various settings, including home visits.

Qualifications

• Bachelor’s degree in physical therapy (required).
• Master’s or doctoral degree preferred.
• Valid California Physical Therapy License, or proof of license eligibility.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Conduct Physical Therapy assessments in the facility and occasionally in the home.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, providing training and guidance to implement Functional Maintenance Programs.
• Provide training to staff, participants, and caregivers on use and care of therapeutic equipment.

Education

Bachelor’s degree in physical therapy
