Job summary

WelbeHealth is seeking an Occupational Therapist to coordinate restorative and rehabilitative OT services within our interdisciplinary team, maximizing participant independence and safety in ADLs. You will work closely with the PACE Physician and rehab staff to deliver comprehensive, patient-centered care.

Qualifications

• Bachelor's degree in occupational therapy; master's preferred.
• New Jersey OT license or eligibility.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct occupational therapy assessments in facility and occasionally in the home to define participant care needs.
• Actively participate in IDT meetings to develop care plans.
• Deliver and document interventions as per care plans.
• Provide active and passive exercise, gait and ADL training.
• Train Therapy Assistants to implement Functional Maintenance Programs.
