Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and licensure maintenance.
• CMS-compliant documentation in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents under supervision.
• Conduct screenings to determine need for intervention.
• Assist in evaluations and treatment planning with the Physical Therapist.
• Carry out individual and group treatment per the treatment plan.
• Monitor resident response and suggest modifications as needed.
• Coordinate resident schedules and participate in QA activities.
• Complete compliant documentation in NetHealth EMR per CMS expectations.
