Job summary

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill for services according to CMS expectations and company policy.
• Evaluate, plan, implement and follow up resident therapy programs.
• Screen residents at regular intervals to determine need for intervention.
• Develop treatment plans with short and long-term goals; communicate results to stakeholders.
• Perform all duties to discharge responsibilities in the best interests of the organization.
