Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Reading, MA. The role supports resident care and therapy services with adherence to standards and CMS guidelines.

The position requires a BS in Physical Therapy, an active PT license, and ongoing continuing education. It involves direct patient care, documentation, and collaboration with an interdisciplinary team in a full-time or PRN capacity.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services per established standards and procedures.
• Document and bill therapy services accurately.
• Evaluate residents and develop short/long-term treatment goals.
• Conduct screenings to determine intervention needs.
• Communicate findings to residents, families and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.

Education
