Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist Assistant to provide in-house skilled nursing facility therapy in Reading, MA, under the supervision of a Physical Therapist.

Responsibilities include conducting screenings, contributing to evaluation and treatment planning, and delivering individual and group therapy focused on strength, mobility, gait, ROM and balance. CMS-compliant documentation in NetHealth EMR is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Continued education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine intervention needs.
• Assist in evaluation, goals, and treatment plans with the Physical Therapist.
• Deliver individual and group therapy per treatment plan including ROM, strength, gait and transfer training.
• Train residents, families and staff in equipment use and care.
• Document compliant and medically necessary CMS-related records in NetHealth EMR.
• Support scheduling and coordination with the care team to maximize productivity.
