Job summary

Evolve Therapy Services in Reading, MA, is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to provide resident-focused care and help maintain high-quality services in a supportive team setting.

Responsibilities include screenings, treatment planning, group and individual sessions, and education for residents, families, and staff.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying a target list with short-term and long-term goals and treatment plans.
• Conduct both individual and group treatment as assigned, including strength, mobility, gait, ROM and modalities.
• Assist in selecting techniques/media and sequence activities based on resident performance.
• Monitor residents' response to intervention and consult with the Physical Therapist if modification is indicated.
• Establish and modify resident schedules with the PT and other team members.
• Adhere to department and resident schedules and adjust as needed for treatment regimens.
• Train residents, family, and staff in the use of equipment.
• Participate in resident, family, and staff education.
• Document in CMS-compliant NetHealth EMR systems.
