Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Reading, MA. You will work under the supervision of a Physical Therapist to deliver therapy, conduct screenings, and help develop treatment plans to improve residents’ mobility and function.

The role involves both individual and group therapies, documentation to CMS standards, and ongoing professional development. Full-time or PRN options available with competitive benefits.

Qualifications

• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct resident screenings to determine intervention needs.
• Assist in evaluating residents and formulating treatment plans with the Physical Therapist.
• Lead individual and group treatments including strength, mobility, gait, balance, ROM and modalities.
• Assist in selecting PT techniques and adapt per resident performance.
• Monitor responses to therapy and modify as directed by PT.
• Participate in scheduling and coordination with the care team.
• Deliver resident and family education and documentation per CMS expectations.
