Evolve Therapy Services in Reading, MA is seeking a compassionate Physical Therapist Assistant to support an in-house Skilled Nursing Facility. You will assist the Physical Therapist, conduct screenings, and help implement treatment plans in a resident-centered program.

This role requires an Associate's degree in physical therapy and an active PTA license. Ongoing education is encouraged; full-time opportunities with competitive pay and benefits are available.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure/certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list, including short-term goals, long-term goals and treatment plan.
• Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in treatment plan (area: strength, mobility, gait, ROM, etc.).
• Assist in the selection of physical therapy techniques/media and sequencing of activities per resident performance.
• Monitor residents’ response to intervention and consult with the Physical Therapist for modifications.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and team.
• Adhere to department and resident schedule and adjust for treatment regimen.
• Train resident, family and appropriate staff in equipment use/application.
• Participate in resident, family, and staff education.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Report to work on time and coordinate schedule to maximize productivity.
• Assist with Quality Assurance Program as required.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
