Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Natick, MA. The role requires working under the supervision of a Physical Therapist and delivering resident-centered PT services to improve function and mobility.

Responsibilities include screenings, treatment planning, group sessions, and staff/family education. Licensure and ongoing professional development are required to maintain credentialing.

Qualifications

• Associate's degree in physical therapy from an accredited program.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in developing and following treatment plans with short- and long-term goals.
• Conduct individual and group treatment focusing on strength, mobility, balance, gait, ROM, and modalities.
• Monitor residents' response to interventions and modify as advised by the Physical Therapist.
• Help schedule residents with the Physical Therapist and other team members.
• Train residents, families, and staff in use of equipment.
• Participate in resident and family education and ensure documentation complies with CMS expectations.
