Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Reading, MA. The role supports PTs, conducts screenings, assists in treatment planning, and delivers individual and group therapy focusing on mobility, gait, balance, and ROM.

Requirements include an accredited Associate's degree in physical therapy and an active PTA license in the practicing state.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in treatment planning and goal setting with the Physical Therapist.
• Conduct individual and group treatment focusing on mobility, balance, gait, ROM and related modalities.
• Train residents, families and staff in use of equipment.
• Maintain compliant documentation per CMS expectations in NetHealth EMR.
