Job summary

Evolve Therapy Services, LLC in Reading, MA is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility. This full-time position partners with a Physical Therapist to deliver resident-focused therapy services and maintain CMS-compliant documentation.

You will conduct screenings, assist in evaluation, set goals, lead individual and group treatments, and participate in resident education and care planning.

Qualifications

• Associate's degree in Physical Therapy from an accredited program
• Active PTA license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and goal-setting with the Physical Therapist.
• Participate in development of treatment plans and duration.
• Lead individual and group therapy sessions per plan.
• Document compliant and medically necessary care in NetHealth EMR.
