Job summary

Evolve-Therapy-Services is seeking a Physical Therapy Assistant for an in-house skilled nursing facility to support licensed Physical Therapists in delivering patient-centered care.

The role involves screenings, treatment planning, and carrying out plan-directed therapy, documenting progress in NetHealth EMR, and educating residents and families. An Associate's degree and active PTA license are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in targeting goals and treatment plans with the Physical Therapist.
• Conduct individual and group treatments per the treatment plan (strength, mobility, gait, ROM, modalities).
• Assist in selecting techniques/media and adjust as indicated by resident performance.
• Monitor residents’ response to intervention and consult with the Physical Therapist for modifications.
• Assist in scheduling and coordination with the team.
• Train residents, families, and staff in equipment use.
• Participate in resident, family, and staff education.
• Ensure documentation per CMS expectations in NetHealth EMR.
