Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant to provide in-house skilled nursing facility therapy on a PRN basis. You will work under the supervision of a Physical Therapist to deliver targeted interventions, screenings, and treatment plans for residents.

You will participate in team education, adjust plans as needed, and help maintain compliant CMS documentation within NetHealth EMR. Strong communication and collaborative care are essential in this role.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and treatment planning with the Physical Therapist.
• Deliver individual and group therapy sessions per treatment plan.
• Maintain resident schedules and coordinate with the care team.
• Document CMS-necessary activities in NetHealth EMR.
