Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapist Assistant to join our in-house skilled nursing facility team. The PTA will provide therapy services under the direct supervision of a Physical Therapist and help plan and implement evidence-based treatment programs for residents.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under the direction of the Physical Therapist.
• Assist in establishing target short-term and long-term goals and treatment plans.
• Conduct individual and group treatment addressing strength, mobility, balance, gait, ROM and modalities.
• Assist in selecting techniques/media and adapt plans in coordination with the Physical Therapist.
• Monitor residents’ responses and consult with the PT for modifications.
• Assist in resident schedules with the Physical Therapist and team members.
• Maintain adherence to department and resident schedules and adjust as needed for treatment regimens.
• Train residents, families, and staff in equipment use and application.
• Educate residents, families, and staff about therapy plans.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license

Continuing education to maintain licensure
