Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant to provide in-house skilled nursing facility services under the supervision of a Physical Therapist. The role supports resident care planning, screenings, and treatments across mobility, strength, gait, and function.

Responsibilities include scheduling with the PT and team, documenting CMS-compliant records, and participating in ongoing education to maintain licensure.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying a target list, including short-term goals, long-term goals and treatment plan.
• Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in treatment plan.
