Job summary

LiveWell Homecare Agency, LLC is seeking a Physical Therapist to provide in-home, senior living, and virtual care for older adults. The role involves evaluating patients, creating personalized plans, and delivering evidence-based treatments to enhance mobility and independence.

You will document care in EMR, collaborate with an interdisciplinary team, and educate patients and caregivers on safety and aging in place. Telehealth is used to extend access and continuity of care.

Qualifications

• Valid PT license or eligibility in the practice state.
• Degree from an accredited program, with basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and driving.
