Qualifications

• Valid Physical Therapist license and ongoing credentialing.
• Degree from an accredited PT program.
• Proficiency with computers and EMR systems.
• CPR certification required.

Responsibilities

• Conduct evaluations and develop personalized care plans.
• Deliver one-on-one therapy sessions in home and senior living settings.
• Document patient progress in EMR and maintain records.
• Collaborate with interdisciplinary teams to optimize care.
• Educate patients and caregivers; incorporate telehealth when appropriate.
