Job summary

Alpha Home Health and Hospice in Virginia is seeking a Licensed Physical Therapist to provide one-on-one care to older adults in private homes and senior living communities, using evidence-based treatment plans to improve mobility, independence, and quality of life.

Role: Licensed Physical Therapist providing one‑on‑one care to older adults in private homes and senior living communities, utilizing evidence‑based treatment plans to improve mobility, independence, and quality of life.

• Document care accurately within the EMR system complying with regulatory standards.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging‑in‑place strategies.
• Incorporate telehealth and virtual care to enhance access and continuity of care.

Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to patient locations, while wearing PPE when necessary.
