Job summary

LiveWell Homecare Agency, LLC is seeking a Licensed Physical Therapist to deliver personalized in-home care for older adults, with a flexible schedule and telehealth options.

You will evaluate patients, design and implement treatment plans, and document care in EMR while collaborating with a multidisciplinary team to promote mobility and independence in private homes and senior living communities.

Qualifications

• Valid PT license
• Degree from an accredited PT program
• Proficiency with EMR and computers
• Current CPR certification
• Ability to meet physical demands (lifting/assisting patients)

Role:

Licensed Physical Therapist providing personalized care to older adults in private homes and senior living communities, utilizing a flexible schedule and telehealth options.

Responsibilities:

Conduct evaluations, develop and implement individualized treatment plans, deliver evidence-based therapy to promote mobility and independence, document care in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and support aging in place through virtual and in-person sessions.

Qualifications & Skills:
