Job summary

LiveWell Homecare Agency, LLC seeks a dedicated Physical Therapist to provide one-on-one geriatric rehabilitation in private residences and senior living communities. You will focus on improving mobility, independence, and quality of life through evidence-based treatment and thorough evaluations.

You will document in EMR systems, collaborate with an interdisciplinary team, and educate patients, caregivers, and staff on safety and aging in place. Telehealth options enhance access.

Qualifications

• Licensed physical therapist or eligible to obtain licensure in the state.
• Degree from an accredited physical therapy program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Responsibilities

• Deliver one-on-one geriatric therapy sessions in homes or senior living settings.
• Develop and implement individualized care plans.
• Document care effectively within EMR systems.
• Collaborate with PTs, OTs, SLPs, and fitness staff.
• Educate patients and caregivers on safety and aging in place.
• Incorporate telehealth and virtual care to extend access.

Responsibilities:

• Deliver personalized therapy sessions supporting patient wellness and functional independence
• Develop and implement individualized care plans
• Document care effectively within EMR systems
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Incorporate telehealth and virtual care to extend access and flexibility

• Valid state Physical Therapy license or eligibility
• Degree from an accredited program
• Proficiency in computer and EMR systems

Physical & Other Requirements:

• Ability to exert medium physical effort, lift up to 50 lbs, and assist with patient transfers
• Repositioning, standing, walking, and climbing stairs
• Driving to patient locations as part of care delivery
• Wearing PPE when necessary
