- Perform evaluations, develop personalized treatment plans, and deliver evidence-based therapy
- Support patient mobility, independence, and wellness through therapy sessions
- Document accurately using EMR systems in compliance with regulations
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff to promote safety and aging in place
- Utilize telehealth and virtual care to enhance access and flexibility
Role & Responsibilities

Qualifications & Skills
- Valid Physical Therapy license or eligibility in practice state(s)
- Degree from an accredited PT program
- Proficiency in computer use and EMR documentation
- Current CPR certification
Key Duties & Physical Requirements
- Exert medium effort, including lifting up to 50 lbs occasionally
- Perform CPR and assist with patient transfers and mobility
- Reposition patients, stand/walk for extended periods, climb stairs
- Drive to patient locations and wear PPE as needed
