Health Marketing is seeking a licensed Physical Therapist to deliver outpatient rehabilitation services primarily in patients' homes for older adults. You will assess, plan, and implement therapy programs to promote independence, mobility, and overall well‑being.
Responsibilities include evaluating patients, developing treatment plans, supervising support staff, documenting treatments, and educating patients and caregivers on mobility techniques.
The Physical Therapist provides outpatient rehabilitation services primarily in-home for older adults, assessing and developing individualized therapy programs to promote independence and improve movement, breathing, and overall well-being. They evaluate patients, establish treatment plans, and collaborate with a multidisciplinary team, adjusting care as needed. Responsibilities include supervising support personnel, adhering to schedules, documenting treatments, and ensuring patient safety. The therapist also orders supplies, maintains equipment, and educates patients and caregivers on health management and mobility techniques. Additionally, they perform functional assessments, fabricate adaptive devices, and incorporate wellness strategies into care plans.
Candidates must hold a valid Physical Therapy license in their practice state(s), possess a Master’s degree in Physical Therapy or equivalent, and have reliable transportation. Experience requirements vary from educational credentials to years of progressive PT practice. Strong communication, adaptability, and patient-centered care are essential.
