AVIR at Alice is seeking a compassionate and dedicated Per Diem Physical Therapist (PT) to join our in-house rehabilitation team.
This is an excellent opportunity for a Physical Therapist who enjoys providing quality, patient-centered care and collaborating with an interdisciplinary team to help residents improve mobility, regain independence, and achieve their functional goals.
Position Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans.
- Provide skilled physical therapy services based on residents' needs, goals, and established plans of care.
- Implement therapeutic exercises, mobility training, balance activities, gait training, transfer training, and functional activities.
- Monitor residents' progress and modify treatment plans as appropriate.
- Provide education to residents and caregivers regarding mobility, safety, therapeutic exercises, and functional activities.
- Collaborate with PTAs, OTs, COTAs, SLPs, nursing staff, physicians, and other members of the interdisciplinary team.
- Complete accurate and timely clinical documentation.
- Participate in care planning and communicate residents' progress, needs, and recommendations to appropriate team members.
- Maintain compliance with facility, state, and federal regulations.
Qualifications
- Current Texas Physical Therapist (PT) license in good standing.
- Graduate of an accredited Physical Therapy program.
- Strong clinical, communication, and organizational skills.
- Experience in skilled nursing, subacute rehabilitation, or a similar setting preferred.
- Compassionate, professional, and resident-focused approach to care.
