Job summary

AVIR at Alice is seeking a compassionate Per Diem Physical Therapist to join our in-house rehabilitation team at 800 Coyote Trail, Alice, TX 78333. You will evaluate residents and develop individualized PT plans to improve mobility and independence.

Collaborate with PTAs, OTs, COTAs, SLPs, nursing staff and physicians; document accurately, participate in care planning, and ensure compliance with state and federal regulations. New graduates are encouraged to apply.

Qualifications

• Current Texas PT license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and organizational skills.
• Experience in skilled nursing or similar setting preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services based on residents' needs, goals, and established plans of care.
• Implement therapeutic exercises, mobility training, balance activities, gait training, transfer training, and functional activities.
• Monitor residents' progress and modify treatment plans as appropriate.
• Provide education to residents and caregivers regarding mobility, safety, therapeutic exercises, and functional activities.
• Collaborate with PTAs, OTs, COTAs, SLPs, nursing staff, physicians, and other members of the interdisciplinary team.
• Complete accurate and timely clinical documentation.
• Participate in care planning and communicate residents' progress, needs, and recommendations to appropriate team members.
• Maintain compliance with facility, state, and federal regulations.
